Academic Matters

  • Adjunct Form [PDF]
    • Form to be completed each semester by department chair/program director, providing data for all adjuncts appointed to teach in the department/program for that semester
  • Annual Department Report Form [PDF]
    • To be completed by department chairs when submitting annual department report to the dean; Reports are to be submitted electronically by the due date determined by the dean
  • Advising Schedule Form [PDF]
    • To be completed by departments indicating advisement schedules for the semester
  • Authorization for Tutorials Form [PDF]
    • To be completed by faculty member and department chair when requesting dean’s approval for (a) course to be taught as a tutorial or independent study or (b) master thesis or doctoral dissertation supervision; form needs to be signed and dated by faculty member and department chair prior to submission to the dean for approval
  • Chairperson Self-evaluation Form
    • To be completed by department chair at the end of each Spring semester. Due in Dean's Office by June 10, with conference to follow.
  • Computer Lab Request Form
    • Form to be submitted to Registrar to request assignment of an existing course to a computer lab in the Academic Computer Center; form must be signed and dated prior to submission to the Registrar.
  • Course Modification Form
    • Form to be completed by department chair or program director to delete, modify, or cancel a course; the form must be signed and dated prior to submission to the dean's office.

  • Lecture Hall Request Form
    • Form to be submitted to the Registrar's office to request assignment of an existing course to a lecture hall; the form must be signed and dated prior to submission to the Registrar.
  • Request to Add Course
    • Form to be completed by the chair or program director to request the addition of a course; form must be signed and dated prior to submission to the dean
  • Request for Overload Form [PDF]
    • Form to be completed by department chair when requesting permission for faculty member to teach an overload (work beyond one’s annual contractual obligation for which compensation is deferred); justification must be provided and the form must be signed and dated by faculty member and department chair prior to submission to the dean

Personnel Matters

  • Annual Faculty Evaluation Form
    • Template to be used by department chair or designee for annual faculty evaluation; form is to be signed by faculty member who can provide comments and chair; original is to be filed in department and copy provided to faculty member
  • College Assistant Timesheet Form [PDF]
    • Biweekly cumulative workload report to be completed for college assistants; must be signed by college assistant and supervisor and submitted to HR according to the time-sheet submission deadline schedule [PDF]
  • CUNY Curriculum Vitae Form [PDF]
    • Lehman College curriculum vitae template (CUNY CV): Curriculum Vitae should be maintained and kept current using the CUNY format, which is required for all personnel actions including tenure and promotion. Department Chairs should receive updated copies of the CUNY CV at the time of the annual evaluation.
  • Four Day Work Schedule Form [PDF]
    • Form to be completed and signed by the department chair delineating summer work schedules of instructional and non-instructional staff
  • Full-Time Position Request Form
    • Form to be completed and signed by the department chair/director before submitting to the dean for approval
  • HEO Performance Management Form [PDF]
    • HEO performance evaluation form to be completed by supervisor/manager for HEOs who have not been granted CCE; form to be signed and dated by HEO and supervisor/manager prior to submission to dean

    • Monthly instructional staff attendance report to be completed and signed by department chair or supervisor

    • Form to be submitted to the dean outlining faculty reassigned time plan (on a partial, annual, or full 3-year basis); must be signed and dated by faculty member and chair prior to submission to dean

    • Form (in Word format) to be completed for summer teaching and nonteaching assignments and signed by faculty member and chair.

Faculty Development

  • CUNY Compact Travel Authorization Form [PDF]
    • Form to be completed by junior faculty to request reimbursement for attendance at professional conferences; the form must be signed and dated by faculty member and chair prior to submission to the dean's office.
  • Online Teaching and Learning Request for Proposals - RFP [PDF]
    • Forms to be completed by faculty interested in submitting a proposal to participate in the Online Teaching and Learning workshop series offered in the School of Arts and Humanities; forms must be signed by the applicant and Department chair and submitted to the dean's office by the due date specified in the RFP.
  • Request for Travel Authorization Form [PDF]
    • Form to be completed by faculty member in order to be considered for reimbursement for anticipated annual travel to professional meetings; completed forms must be signed and dated by faculty member and department chair prior to submission to the dean’s office; forms must be received in the dean’s office by the specified deadline in order to be eligible for reimbursement
