THE FOLLOWING ARE THE REQUIRED DOCUMENTATION FOR REGISTRATION ELIGIBILITY

1. Proof of Employment Eligibility – (MANDATORY) – One of the following:
   • Birth Certificate (Only born in USA and Puerto Rico)
   • US Passport
   • Resident Alien Card – not expired
   • Naturalization Certificate
2. Social Security Card – (MANDATORY)
3. Name Change – if the names on documentation are not the same - One of the following:
   • Marriage Certificate
   • Legal name change documentation
4. Proof of Employment – (If Applicable) Pay Stubs
5. Income Verification – (If Applicable)
   • Public Assistance Budget letter
   • Food Stamp Budget Statement letter
6. Proof of Unemployment Status – (If Applicable) – One of the following:
   • Substantial layoff notice stating plant closing, substantial layoff of at least 33% of workforce from your previous employer
   • Individual termination/layoff letter stating the last day of work and job title on company letterhead from your current or previous employer
   • Proof of collecting or collected Unemployment Insurance: Original Benefit Letter, Unemployment pay stub or Unemployment print out.
7. Selective Service – All males born after January 1, 1960 must be registered with Selective service
8. Resume – If you do not have one, we will assist you in developing a resume
