Students can now submit a Meningococcal Meningitis Response Form Online !Since August 15, 2003, the New York State Public Heath Law 2167 has required that all students must complete and submit the Meningococcal Meningitis Response Form to the College Health Center. Students who do not file a Meningitis Response Form will be blocked from registration and attending classes. Students are NOT required to obtain the meningococcal vaccine, but they are required to submit the form.

How do I know if I need to fill out a Meningococcal Meningitis Response Form?
Students without a Meningitis Response Form on file will receive a Meningitis Stop (MG). You will be alerted to the lack of a Meningitis Response Form as follows:
- On eSIMS Web Registration, when the "Registration" menu item is selected, a warning notice will appear as shown below stating "Click here
to complete a Meningitis Response before registering" (see illustration).

How to fill out the Meningococcal Meningitis Response Form online using eSIMS.

STEP 1: Log onto your eSIMS account using your 9-digit SSN and then a PIN number.
STEP 2: Scroll down the left side menu and mouse over the 'Meningitis Check-off' menu label and click to proceed.

*Remember, you will NOT be able to register with a Meningitis Stop.*
|

Also, when registration is active, you can scroll down the left side menu and mouse over the 'Registration' menu label. On the right side of the registration label, a message box will appear stating “Click Here to complete a Meningitis Response before registering”. Click on the message box to proceed.

STEP 3: The Meningococcal Meningitis Response Form will appear. Please read the form thoroughly before proceeding to the next step.

STEP 4: Choose one (1) out of the two radio buttons that conforms to your Meningitis Vaccine status.
STEP 5: Enter the date immunization was received, if applicable. If no vaccine was ever received, leave blank.

STEP 6: Enter your birth date for verification. Birth date is REQUIRED. You cannot proceed if this field is left blank. All dates MUST be entered in MM/DD/YYYY format!
STEP 7: Click on the ‘Submit’ button.
STEP 8: You will receive a confirmation notice as shown and may proceed with registration.

Having a problem submitting the form?
After clicking the "Submit" button, you may see the following error:

The Birth Date entered on the form is producing an error. If you are entering your birth date in the correct MM/DD/YYYY format and you still receive
an error message, your birth date may NOT be matching what the College has on file in our database.
Proceed to verify your birth date at the Registrar's Record Office located in Shuster Hall, rm. 106. You MUST produce VALID ID before office personnel
will verify your student records.
