    The Records Unit maintains and processes all information pertaining to a student's academic record. This includes personal information, course registrations, grades, major/minor, transfer credit and credit taken on permit.

    Declaration of Major and Minor

    A declaration of major and/or minor form must be filed. Student must declare a major by the time they have earned 60 credits and minor by the time they have earned 80 credits toward their degree. A completed form, signed by the department advisor in which the student wishes to major/ minor, should be returned to Shuster Hall, rm. 106.

    Candidates for a second bachelor's degree must also file a second degree area of concentration form. All forms are available in Shuster Hall, rm. 106 during normal business hours. This form is also available in Shuster Hall, rm. 114 during evening hours when classes are in session.

    Change of Name or Other Personal Information

    A change of name may be submitted by completing a Change of Name form, accompanied by appropriate original legal documentation such as a court order, marriage certificate or annulment/divorce papers. All other changes also require original documentation.

    Change of Address

    Students may change their address by submitting a form in Shuster Hall, rm. 106 or 114. This form is only available in Shuster Hall, rm. 114 during evening hours when classes are in session.

    First semester students should report to the Admissions Office, Shuster Hall, rm. 161 to change their address. Students moving out of state, foreign students and students applying for a change of residency must report changes to Shuster Hall, rm. 182.

    Permit Update

    Beginning with Spring 2005 Registration, ePermit will be the only way to apply for a permit. In-person, paper permits are no longer available. For more information on epermit, click HERE.

    Status of Grade Changes, Departmental Recommendations and Academic Exceptions

    Currently enrolled students may check the status of grade changes and academic exceptions by logging on to eSIMS.

    If a grade change or academic exception has not been processed, or a student is not currently enrolled, further information may be obtained in Shuster Hall, rm. 106. Photo ID is required.
